ADOPTION/RESCUE AGREEMENT

RESCUES MUST FAX THE ADOPTION AGREEMENT FOR THE ANIMALS. THE TWO PAGE ADOPTION AGREEMENT IS NECESSARY, NOT JUST A PLAIN PAPER FAX. ALL DOG I.D. NUMBERS MUST BE LISTED ON THE FAX WITH YOUR ACTUAL SIGNATURE TO BE VALID. YOU MUST CALL AND CONFIRM YOUR FAX HAS BEEN RECEIVED
